HOW TO SUBMIT YOUR REQUEST
Before filing out the forms, please make sure you have the scanned copies (pdf or jpeg format) of the required documents with you, as you will be required to upload during the online fill-out process.
Please follow our format of the required documents before uploading to our system or submitting to us via email at hello@apostillepinoy.com. Kindly use a high resolution scanner, or when using your phone's camera, use "Microsoft Lens" or CamScanner" to scan your required documents.
1. Unexpired PRC PIC (Professional Identification Card) - Front and Back
2. Transcript of School Records (TOR)
3. Unexpired Philippine Passport Bio Page - to prove that the Professional is still a Philippine citizen
4. Certificate of Registration (COR) or Lupon
5. Certificate of Rating
6. State Board Verification Request Form issued by the requesting governing body
7.Handwritten authorization letter / request (a template is provided once all initial documents are reviewed)
8.Photograph (i) holding and (ii) signing the handwritten authorization letter
After filling out the online forms, you will be prompted to make a payment.
For professionals who have lost their Filipino citizenship, the PRC shall issue a Confirmation of Professional Registration in lieu of Stateboard License Verification. You may proceed with our online request form and submit the following documents, instead:
1. PRC PIC (Professional Identification Card) - Front and Back, expired or unexpired
2. Passport or any valid government ID, indicating the new citizenship
3.Handwritten authorization letter / request (a template is provided once all initial documents are reviewed).
4.Photograph (i) holding and (ii) signing the handwritten authorization letter
NOTE: All documents are now required to be delivered to us which in turn will be submitted to the PRC.
